Patient Payment Policy

Thank you for choosing our practice! We are committed to the success of your treatment and care. Please understand that payment of your bill is part of this treatment and care. For your convenience, below is a list of commonly asked financial policy questions. If you need further information, please ask to speak with the billing specialist or practice manager.

What Payment Methods Do You Accept?

We accept payment by cash, check, Visa, Mastercard and American Express.

Do I Need a Referral?

If you have an HMO or Managed Care Plan with which we are contracted, you need a referred from your primary care physician prior to being seen in our office. Please fax it ahead of your visit (310.677.4211) and call to confirm receipt of your fax (310.672.9999). If we have not received an authorization prior to your arrival, we have a telephone for you to call your PCP to obtain it. If you are unable to obtain at that time, your visit will be rescheduled.

Procedure

If your physician recommends a procedure, you will be seen by one of the front office coordinators who will get patient education material together about the proposed procedure and call you later in the day to schedule the procedure. The coordinator will discuss the paperwork and associated preprocedure tests involved. Our office will complete pre-certification or authorization for the procedure if your insurance requires it.

Which Insurance Plans Do You Contract With?

We are contracted with most major managed care including but not limited to Blue Cross, Blue Cross/Blue Shield, HealthNet, Benesight, United Healthcare, Cigna and PacificCare.

What Is My Financial Responsibility for Services?

Your financial responsibility depends on a variety of factors.

IF YOU HAVE YOU ARE RESPONSIBLE FOR OUR STAFF WILL
Commercial Insurance aka indemnity, regular or PPO Payment of the deductible or copay at time of service. If there is a remaining balance per insurance explanation, a statement will be sent allowing 30 days to pay.

Call your insurance company ahead of time to determine deductibles and coinsurance.

File an insurance claim as a courtesy to you.

HMO and Medi-cal

If services you receive are covered by your plan.
All applicable copays and deductibles are due at time of service.

If services are not covered, payment in full is requested.

HMO for which we are not contracted

Obtaining an out of plan authorization from your primary care provider. You will be responsible for any copays or deductibles.

If out of plan authorization does not exist, you are responsible for payment in full for all office visits, injections, lab draws and procedures.

Arrange for a letter of agreement with the hospital if you need a procedure.

Provide information for you to complete and file a claim with your insurance company.

Point of Service Plan or Out of Network PPO You will be responsible for all deductibles,
copays and non-covered services.

Call your insurance company ahead of time to determine copays, deductibles and non-covered services for you.

File an insurance claim as a courtesy to you.

Medicare

If you have regular Medicare, and do not have a secondary that will cover your deductible have not met your $110 deductible, we ask that it be paid at time of visit.

If you have Medicare and a secondary or Medi-cal – no payment is due at time of visit.

If you have Medicare and no secondary, you will be billed for the 20% balance as indicated by the Medicare Explanation of Benefits. Payment is due in 30 days.

We will courtesy bill you for the deductible if applicable.

File all insurance claims for you as well as any claims to your secondary insurance.

Medicare HMO You will be responsible for all deductibles,
copays and non-covered services.

Workers Compensation

If we have verified with your carrier, no payment is necessary at time of visit.

If we have not verified your claim, payment in full is due at time of visit.

Call your carrier ahead of time to verify the accident date, claim number, primary care physician, employer information and referral procedures.
No Insurance Payment in full is due at the time of visit We will work with you to settle your account.
